Registration Documents Needed

Please click on the tabs below to see what documentation is needed for registration.
The person registering the student must be listed on the birth certificate or must have proof of guardianship.

The following forms are accepted:

The name of the parent must be on the proof of residency.  Any proof must be current (last 30 days) and in good standing.

  • Utility Bills: gas, water, electric
  • Lease Agreement 
  • Tax Office Statement

Cable and cell phone bills are NOT considered proof of residency.

If you are living with someone, please contact the office of Steven English at (903) 295-5031 to set up an appointment to complete the Living with Someone Affidavit.

The social security number is used to verify the students identify and will remain confidential.  A 5 digit school student ID number will be issued for school use.

We will accept the from page of the Tax Statement that has the social security number printed on it.

Only a legal parent/guardian may register a student. Please any custody papers if the legal parent/guardian is not listed on the birth certificate.

Accepted ID Cards:

  • Driver's License
  • State Photo ID
  • Passport
  • Consulate Card
  • Military ID- we will not make a copy

If you are joining us from another school, please bring the withdrawal forms and any school records you have.  This will help speed up student placement.

Inform staff that you are moving from another school so that all records can be requested.

Required Documents for all Prekindergarten Students

Proof of Income:

  • Current Pay Stubs of all working in the household OR
  • Current Medicaid Benefits Letter OR
  • Current Supplemental Nutrition Assistance Program (SNAP) benefits letterOR
  • Temporary Assistance for Needy Families (TANF) benefits letter

Program or insurance cards are NOT considered proof of income.

Additional Prekindergarten Qualifying Documents as Applicable

Limited English Proficient: Testing will be scheduled.  LPAC committee will determine eligibility based on test results.

Homeless: Completion of Student Residency Questionnaire

Military: DoD photo ID OR Statement of Service OR Letter from Veteran's Affairs stating eligibility for disability OR Letter from Commanding Officer

Foster Care: DFPS Verification Letter of PK Eligibility OR  other documentation of DFPS conservatorship

Star of Texas Eligible Parent: Certificate OR  Letter from Governor or State Representative
